It’s a washlet bidet, which attaches either underneath the toilet seat or as part of the toilet seat itself, and usually has controls that jut out to the right of the seat(to the right from the perspective of the seated person).

The Japanese toilet uses wash sprayers to act like a bidet. Takes up less room. For some reason, Americans like dirty asses.
